Immunisations
We at Eastlink Family Medical believe that immunisations are a safe and effective way of protecting our patients and the wider community against harmful infections and reducing the spread of diseases.
Children
Children’s immune systems are still developing at a young age, and hence require specific immunisations to protect them from certain infections and diseases.
Child immunisation programs are for birth, 2 months, 4 months, 6 months, 12 months, 18 months and 4 years.

If you have any questions, enquiries or would like to book in your child for their vaccines, please do not hesitate to book an appointment with your GP.
Adults
Adult immunisation requirements can vary depending on age, ethnicity, health history, occupation, travel plans and previous immunisation history.
Influenza Vaccine
We do both government eligible and private administration of the Flu vaccine. Therefore, it is free of cost from 6 months to 5 years of age and adults 65+ years old.
Pneumococcal Vaccine
This is recommended and provided free of charge from 65 years of age. We encourage you to discuss further with your health professional.
Shingles Vaccine
This is recommended and provided free of charge at 70 years of age unless contraindicated. We encourage you to discuss further with your health professional.
Weight Loss
Being a healthy weight is important to prevent illnesses such as high blood pressure, type 2 diabetes, heart disease, stroke, osteoarthritis, and cancer.
Your GP can help you along this journey by assessing your weight, BMI, lifestyle and overall health. They can assist you a specific approach for you, tailored to your specific lifestyle and needs. They can also refer you to a Dietitian, who can assist you in meeting your health goals!
Chronic Disease Management
At Eastlink Family Medical, we aim for you to reach all your health ambitions and live at your optimal wellbeing. We can assist you with your chronic medical condition through management plans and health assessments.
A chronic medical condition is persistent for longer than 6 months and may include:
- Asthma
- Arthritis
- Cancer
- Cardiovascular Disease
- Diabetes
- Hypertension
- Kidney Disease
- Musculoskeletal conditions
GP Management Plans are suitable for patients who are receiving ongoing care and require a structured approach. Your doctor will determine if this is appropriate for you and your needs, and hence your Care Plan will have little to no cost involved.
If you have a chronic medical condition, book an appointment with your doctor to determine if you are eligible for a Care Plan.
Mental Health
Eastlink Family Medical can provide you with a Mental Health Care Plan for the treatment of your mental health over an extended period of time. This allows you to obtain subsidised visits to psychologist. To see if you are eligible, discuss your condition with the GP in your next consultation.
